The Evolution of Spinal Surgery Techniques

23 June 2024

Spinal surgery has moved a long way from the large midline incisions and extended hospital stays that once defined it. Microscopic and minimally invasive techniques now allow many procedures — decompression for a narrowed spinal canal, discectomy for a slipped disc, even fixation for an unstable spine — to be performed through incisions of a centimetre or two.

The principle behind this shift is straightforward: the less muscle and soft tissue disturbed to reach the problem, the less pain and the faster the recovery afterward. Techniques like tubular retractors, percutaneous screw fixation and endoscopic discectomy have made this possible for a growing share of spinal conditions that used to require open surgery as a matter of course.

Not every spinal condition is suited to a minimally invasive approach — complex deformities and some tumours still need open surgery to be done safely — but for the common problems that bring most patients to a spine surgeon, the trend has clearly been toward smaller incisions and quicker returns to daily life.
